Bone marrow is tissue located in the center of your bones, making healthy blood cells that strengthen your immune system and fight off outside infections. A large amount of cells are located in bone marrow, and doctors frequently use hip bone marrow for most transplants, since the stem cells in this area are the most plentiful.
If someone doesn’t have cord blood stored, they will have to rely on stem cells from another source. For that, we can go back to the history of cord blood, which really begins with bone marrow. Bone marrow contains similar although less effective and possibly tainted versions of the same stem cells abundant in cord blood. Scientists performed the first bone marrow stem cell transplant in 1956 between identical twins. It resulted in the complete remission of the one twin’s leukemia.

The work from Dr. Verfaillie’s lab on the multipotent adult progenitor cell (MAPC) has received much attention (15,16,18–22). Their findings indicate that the MAPC is pluripotential and slightly enigmatic, as it appears after extensive passage in cell culture. Similarly, in umbilical cord blood, Kogler et al. (17,23) identified a cell that they call the universal somatic stem cell (USSC). The USSC is another rare cell (average of 16 cells in initial isolate; able to isolate USSC in 50% of the cords attempted). The USSC, like the MAPC, offers much promise as an embryo-safe pluripotent cell. Widespread acceptance of these two cells will come when the methods for their isolation become robust such that any laboratory can isolate them and contribute to the field.

MSCs are reported to have immune-suppressive effects. To comment human fetal and adult MSCs are not inherently immunostimulatory in vitro and fail to induce proliferation of allogeneic lymphocytes (37–39; for review, see ref. 40). In one human case, fully mismatched allogeneic fetal liver-derived MSCs were transplanted into an immunocompetent fetus with osteogenesis imperfecta in the third trimester of gestation (41). No immunoreactivity was observed when patient lymphocytes were re-exposed to the graft in vitro, indicating that MSCs can be tolerated when transplanted across MHC barriers in humans. Similarly, after intrauterine transplantation of human MSCs into sheep, the cells persisted long-term and differentiated along multiple mesenchymal lineages (42). Instead, the cells are immunosuppressive and reduce lymphocyte proliferation and the formation of cytotoxic T-cells and natural killer cells when present in mixed lymphocyte cultures. The mechanism whereby MSCs suppress lymphocyte proliferation is still largely unknown but appears to, at least in part, be mediated by a soluble factor. Several factors, including MSC-produced prostaglandin E2, indoleamine 2,3-dioxygenase-mediated tryptophan depletion, transforming growth factor-β1, and hepatocyte growth factor have been proposed to mediate the suppression, but the data remain controversial.
The University of Texas Health Science Center at Houston is conducting a pioneering FDA-regulated phase I/II clinical trial to compare the safety and effectiveness of two forms of stem cell therapy in children diagnosed with cerebral palsy. The randomized, double-blinded, placebo-controlled study aims to compare the safety and efficacy of an intravenous infusion of autologous cord blood stem cells to bone marrow stem cells.
In addition to the stem cells, researchers are discovering specific uses for the other types of cells in the treatment of certain conditions. Cord blood Treg cells hold potential for preventing graft-versus-host disease in stem cell transplantations and ameliorating the effects of autoimmune diseases such as diabetes, rheumatoid arthritis and multiple sclerosis. Cord blood natural killer cells also hold future potential. These cells have been programmed to target specific cancers and tumors in clinical trials. This could make them exceptionally strong candidates for chronic or treatment-resistant cases of cancer.

Parents often complain about cord blood banking costs. This is not an industry where costs can be cut by running a turn-key operation. Each cord blood unit must be individually tested and processed by trained technicians working in a medical laboratory. 
The gene expression analysis and reverse-transcription polymerase chain reaction (RT-PCR) of MSCs from the umbilical cord was reported by one lab using the National Institutes on Aging (NIA) human 15k gene array (28). That work indicated that human UCM cells express genes found in cells derived from all three germ layers to some extent. At least one report indicates that UCM cells express the pluripotency gene markers Oct-4, nanog, and Sox-2 at low levels relative to ESCs (33). One interpretation of these findings is that cord matrix stem cells are pleiotropic and express a relatively large number of genes in relatively low abundance. On the other hand, it may serve as evidence that the cord matrix cell population has a subset of primitive stem cells. Because gene array is not a sensitive method by which to examine low copy number message, we suggest that massively parallel signature sequencing (MPSS) is a more appropriate method of assessing matrix cell gene expression. RT-PCR alone is not useful for characterizing cord matrix stem cells: quantitative RT-PCR is needed to make meaningful statements about gene expression and to compare gene expression between experimental conditions.

There are several cord blood banks that are accredited by the American Association of Blood Banks. Most offer information on cord blood banking and provide private cord blood banking services. With a little research, you should be able to locate a credible cord blood bank online.
Is the blood stored as a single unit or in several samples? Freezing in portions is preferred so the blood can be tested for potential transplant use without thawing — and wasting — the entire sample.

In addition to their immune-suppressive properties, MSCs appear to exhibit a tropism for damaged or rapidly growing tissues. For example, following injection into the brain, MSCs migrate along known pathways when injected into the corpus striatum (44). MSCs migrated throughout forebrain and cerebellum, integrated into central nervous system cytoarchitecture, and expressed markers typical of mature astrocytes and neurons after injection into the lateral ventricle of neonatal mice (45). MSCs injected into injured spinal cord were found to form guiding “cord,” ushering in regenerating fibers (46). MSCs may assist with regeneration in stroke (47–51) or myocardial ischemia (52–55) by release of trophic factors such as brain-derived neurotrophic factor, glial cell line-derived neurotrophic factor, or angiogenic factors (56–61).

Fortunately, those odds should improve soon. In 2005, Congress passed the Stem Cell Therapeutic and Research Act, which provides $79 million in federal funding to create a centralized cord-blood registry much like the one that exists for bone marrow. The goal is to expand the existing inventory of 45,000 donated cord-blood units to 150,000.
There are no hard numbers on a child’s risk of needing a stem-cell transplant: It’s anywhere between one in 1,000 and one in 200,000, according to studies cited by ACOG and the AAP. But private banks’ marketing materials often place the odds at one in 2,700 and note that these numbers don’t factor in its potential future use for diabetes, Alzheimer’s, Parkinson’s disease, and spinal-cord injuries in adults. “Researchers are constantly discovering new treatments using stem cells,” says Gerald Maass, executive vice president of corporate development for Cryo-Cell, a private bank in Clearwater, Florida. Another major bank’s Web site claims incredible odds: “Should cord blood prove successful in treating heart disease, the lifetime probability of being diagnosed with a disease treatable by cord blood will increase from one in 100 to one in two.”

Umbilical cord blood contains a large amount of stem cells. If parents sign up for personalized storage or donation, medical staff will remove stem cells from the umbilical cord and placenta. The blood is then cryogenically frozen, and put into long-term storage.
We will notify both your doctor and the hospital that you have registered to donate your baby’s cord blood to our program. However, we recommend that you remind the Labor and Delivery staff when you go for your delivery that you want to donate cord blood.
Private or family banks store cord blood for autologous use or directed donation for a family member. Private banks charge a yearly fee for storage. Blood stored in a private bank must meet the same standards as blood stored in a public bank. If you have a family member with a disorder that may potentially be treated with stem cells, some private banks will store the cord blood free of charge.
