In fact, the AAP does encourage parents to keep their child’s cord blood if a family member has already been diagnosed with a stem-cell-treatable disease. But a family won’t have to foot the bill: The Children’s Hospital Oakland Research Institute, in California, will bank a baby’s cord blood for free if a family member needs it at the time of the baby’s birth. Some private banks, such as Cord Blood Registry, Cryo-Cell, and ViaCord, have similar programs.
A cord blood industry report by Parent’s Guide to Cord Blood Foundation found that, among developed nations, cord blood banking cost is only 2% of the annual income of those households likely to bank.
Students who register to donate blood three or more times during their high school career earn a Red Cord to wear during graduation events. Seniors must complete the requirement by May 15 (or by the date of their school’s final blood drive of the year, whichever is later).
While the stem cell count is smaller during a cord blood transplant, these cells multiply quickly, and researchers are studying new methods to increase cells naturally. Compared to bone marrow, cord blood cells multiply faster and don’t require an exact match type to complete a successful transplant. Some techniques medical experts are testing to increase the amount of stem cells include:
Even if a sick child has a sibling donor, there’s only a 25 percent chance that cord blood will be a perfect match — and an equal chance it won’t match at all. That’s why public donations are so important. So far, many more stem-cell transplants have been done using cord blood stored in public banks. From 2000 to 2004, more than 2,200 unrelated transplants were done nationwide.
ES cells are pluripotent, and similar to iPS cells, but come from an embryo. However, this kills the fertilized baby inside the embryo. This type of cell also has a high chance for graft-versus-host disease, when transplanted cells attack the patient’s body.
In the event your child becomes seriously ill or develops a genetic disorder, illness affecting the immune system or blood-related disease, please notify the cord blood bank as this could impact the patient receiving the stem cells for transplantation.

The procedure for obtaining the cord blood involves clamping the umbilical cord at the time of birth. The small amount of blood remaining in the umbilical cord is drained and taken to a cord blood bank. It is free to donate.
As the research into umbilical cord blood and it’s therapeutic use for blood diseases has grown, so has the question as to whether people should privately store the cord blood of their offspring for future use. A recent paper on this issue by Mahendra Rao and colleagues advocates the practice of cord blood banking (for treatment of blood diseases) but in the context of public cord blood banks rather than a private cord blood banks. Any adult needing treated would need at least two cord blood samples that are immune compatible. So one sample will not be sufficient. A child might only need one cord blood sample but in the case of childhood leukaemia there is a risk that pre-leukemic cells are present in cord blood sample – and so the child could not use their own cells for therapy.
Even if you don’t want to store the cord blood, highly consider donating the cord blood to local public banks. This cord blood can help patients that are on waiting lists with diseases such as leukemia.
Several groups have isolated MSC-like cells from the umbilical cord tissues or blood and have reported that those cells may express neural markers when differentiated (26,32), and differentiate into neural cells upon transplantation into rat brain. This is not too surprising, because adult bone marrow-derived MSCs injected into fetal rat brain engrafted, differentiated along neural-like lineages, and survived into the postnatal period (34). Similarly, Jiang et al. (19) demonstrated convincingly that bone marrow-derived MAPCs could be differentiated in vitro to become cells with electrophysiological properties of neurons. Increasingly, reports are indicating that bone marrow-derived cells may differentiate, first to neurospheres and then to neurons with proper neuronal electrophysiological characteristics (35,36).
If you’re thinking about banking your baby’s cord blood stem cells, one question you’ve probably considered is whether to choose a private or public cord blood bank. As with any major decision in your life, it pays to do your research so you can make the best choice for your family about the future of your baby’s cord blood.
Much research is focused on trying to increase the number of HSCs that can be obtained from one cord blood sample by growing and multiplying the cells in the laboratory. This is known as “ex vivo expansion”. Several preliminary clinical trials using this technique are underway. The results so far are mixed: some results suggest that ex vivo expansion reduces the time taken for new blood cells to appear in the body after transplantation; however, adult patients still appear to need blood from two umbilical cords. More research is needed to understand whether there is a real benefit for patients, and this approach has yet to be approved for routine clinical use.
Similar to transplantation, the main disadvantage is the limited number of cells that can be procured from a single umbilical cord. Different ways of growing and multiplying HSCs in culture are currently being investigated. Once this barrier is overcome, HSCs could be used to create “universal donor” stem cells as well as specific types of red or white blood cells. Immunologic rejection is a possibility, as with any stem cell transplant. HSCs that are genetically modified are susceptible to cancerous formation and may not migrate (home) to the appropriate tissue and actively divide. The longevity of cord blood HSCs is also unknown.

It also allows us to do the necessary testing to determine whether it will be stored for public banking or made available for research. First, the cells are checked to see if they can be used for a transplant. If there are too few cells, the cord blood unit may be used for research to improve the transplant process for future patients or to investigate new therapies using cord blood, or discarded.

For these and other reasons, the American Academy of Pediatrics (AAP) and many physicians do not recommend private cord blood banking except as “directed donations” in cases where a family member already has a current need or a very high potential risk of needing a bone marrow transplant. The term “cord blood” is used for the blood remaining in the umbilical cord and the placenta after the birth of a baby. Cord Blood contains stem cells that can grow into blood and immune system cells, as well as other types of cells. Today cord blood is often used as a substitute for bone marrow in stem cell transplants. There are over 80 diseases treated this way, including cancers, blood disorders, genetic and metabolic diseases.
Collected cord blood is cryopreserved and then stored in a cord blood bank for future transplantation. Cord blood collection is typically depleted of red blood cells before cryopreservation to ensure high rates of stem cell recovery.

The work from Dr. Verfaillie’s lab on the multipotent adult progenitor cell (MAPC) has received much attention (15,16,18–22). Their findings indicate that the MAPC is pluripotential and slightly enigmatic, as it appears after extensive passage in cell culture. Similarly, in umbilical cord blood, Kogler et al. (17,23) identified a cell that they call the universal somatic stem cell (USSC). The USSC is another rare cell (average of 16 cells in initial isolate; able to isolate USSC in 50% of the cords attempted). The USSC, like the MAPC, offers much promise as an embryo-safe pluripotent cell. Widespread acceptance of these two cells will come when the methods for their isolation become robust such that any laboratory can isolate them and contribute to the field.

Luckily for expectant parents, cord blood can be easily collected at the baby’s birth via the umbilical cord with no harm to the mother or baby. This is why pregnancy is a great time to plan to collect and bank a baby’s cord blood.
People who are in need of a transplant are more likely to find a match from a donor of the same ethnic descent. There are fewer racial minorities in the national registries, so finding a match can be more difficult.5

The cord is cut and clamped, just like normal. The mother doesn’t go through anything different during birth, and neither does her child. They will experience no additional pain or procedures before, during, or after birth.

The longest study to date, published in 2011 by Broxmeyer at al found that stem cells cyro-preserved for 22.5 years engrafted as expected. There was no significant loss of stem cell recovery or proliferation.
