Be the Match is a nonprofit organization that supports public cord blood banks’ efforts to encourage donations. It maintains the largest public listing of donated cord blood available for transplantation in the United States. The organization has facilitated more than 7,000 unrelated cord blood transplants since the year 2000.
Donors to public banks must be screened for blood or immune system disorders or other problems. With a cord blood donation, the mother’s blood is tested for genetic disorders and infections, and the cord blood also is tested after it is collected. Once it arrives at the blood bank, the cord blood is “typed.” It is tracked by a computer so that it can be found quickly for any person who matches when needed.
Parents sign a consent form, giving the public bank permission to add their child’s cord blood to a database. This database will match transplant patients with a suitable donor. No information about the donor, or their family, is displayed online. The website used in America is Be The Match. They maintain a database of donations and banks across the country, while also working with foreign banks. Your child’s cord blood could save someone living anywhere in the world.

However, the American Academy of Pediatrics strongly encourages umbilical cord donations for general research purposes. Donors are encouraged to contact a cord blood bank by the 35th week of pregnancy. 
There is no cost associated with public cord blood banking, but you do give up your rights to your baby’s stem cells at the time of donation. The public cord blood bank owns the donation. If your child or another family member needs a transplant in the future, there is no guarantee you would have access to your baby’s cord blood.
However, parents should know that a child’s own cord blood (stored at birth), would rarely be suitable for a transplant today. It could not be used at present to treat genetic diseases, for example, because the cord blood stem cells carry the same affected genes and. if transplanted, would confer the same condition to the recipient. (See the story of Anthony Dones.) In addition, most transplant physicians would not use a child’s own cord blood to treat leukemia. There are two reasons why the child’s own cord blood is not safe as a transplant source. First, in most cases of childhood leukemia, cells carrying the leukemic mutation are already present at birth and can be demonstrated in the cord blood. Thus, pre-leukemic cells may be given back with the transplant, since there is no effective way to remove them (purge) today. Second, in a child with leukemia, the immune system has already failed to prevent leukemia. Since cord blood from the same child re-establishes the child’s own immune system, doctors fear it would have a poor anti-leukemia effect.
Donating your baby’s cord blood to a public bank is always free. The limitations of the public banking network in the United States are: they only collect donations at large birthing hospitals in ethnically diverse communities, the mother must pass a health screening, they prefer registration by 34 weeks of pregnancy, and they only save the largest cord blood collections. Like any insurance, cord-blood banking isn’t cheap. Banks initially charge from $1,000 to $2,000 to collect and process the stem-cell units, which are stored for a family’s exclusive use. When you factor in additional costs for shipping (about $150 for a medical courier), the doctor’s collection fee (prices can range from $150 to $500), and annual storage fees averaging $100 per year for 18 years, parents can expect to pay up to $4,000 in expenses not covered by insurance.

Donating cord blood to a public cord blood bank involves talking with your doctor or midwife about your decision to donate and then calling a cord blood bank (if donation can be done at your hospital). Upon arriving at the hospital, tell the labor and delivery nurse that you are donating umbilical cord blood.
Most of the diseases on the proven treatment list are inherited genetic diseases. Typically, these treatments require a donor transplant, as from a sibling. Banking a baby’s blood and stem cells in a cord blood bank is a type of insurance. Ideally, you would not need to access your baby’s stem cells in order to address a medical concern. However, using a cord blood bank can provide peace of mind in knowing that you have a valuable resource if you need it.
Osteopetrosis is a genetic disease, so this means that doctors could use a sibling’s cord blood cells to treat Anthony, but they cannot use his own cells because the disease is in every cell in his body. In fact, a majority of the diseases listed in private banking firms’ marketing material as treatable with stem cells are genetic diseases.
Find a public bank that participates with your hospital. Public banks usually partner with specific hospitals, so you will usually only have one choice. If your hospital doesn’t partner with a public bank, or if you don’t like the facility they work with, several private banks offer a donation option, which means public banking may still be possible.
In contrast, although there are several reports of pluripotent cells being isolated from adults (13–17), this work is in need of independent verification. Such verification is important because an alternative source of pluripotent cells, cells derived from adults, offers the best of both worlds: pluripotent cells for therapeutics and cells that are collected with consent from adults (no controversy there).
